There is emblem of the USSR in the middle. It has 7 ribbons (late 1920th - 1936). "Народный комиссариат здравоохранения РСФСР" relates to the part of government of Russian Socialistic Republic been in charge of health care. The commissariats (комиссариаты) later become ministries (in 1946).
